There is one document for use in 46 states which CANNOT be used by nursing home or care facility residents in California, Connecticut, Delaware, Nevada, New York, Oregon and Vermont. If you do not live in a nursing home or other care facility, you can use this form.
New Hampshire, Ohio, Texas and Wisconsin require you to use their own form. These are included here, with life-protecting provisions. You CANNOT use the multi-state form in these four states.
