Life Protecting Power of Attorney for Health Care

This legal document names and empowers another person to make health care decisions for you if you cannot make your own decisions. The requested donation amount is $15. There is one document for use in 46 states which CANNOT be used by nursing home or care facility residents in California, Connecticut, Delaware, Nevada, New York, Oregon and Vermont. If you do not live in a nursing home or other care facility, you can use this form.

New Hampshire, Ohio, Texas and Wisconsin require you to use their own form. These are available, with life-protecting provisions. You CANNOT use the multi-state form in these four states. Once you receive your Power of Attorney document, you may notice that there are portions crossed out or boxes already marked. THIS IS TO MAKE IT LIFE-PROTECTING. These are important legal documents.
